IFC/PHA - Social Event Reporting Form

If you would like to file a complaint against an Interfraternity Council Fraternity or Panhellenic Association regarding the shared IFC/PHA Social Event Policy, please fill out this form. Please note that the form requires your name, phone number, and e-mail address. This information is necessary in order to initiate the investigative process. After your complaint is received, you will be contacted by either IFC or PHA Judicial Officer or both.

This form will likely be seen by Responsible Employees and therefore will be forwarded to the appropriate campus department as needed. Examples include, but are not limited to: Office of Student Conduct & Community Standards, Sexual Misconduct Resource and Response Program
